Spread is applied as half the quoted spread per side (the mid-to-quote distance you cross). A round trip crosses it twice and pays the trading fee twice. Network/withdrawal fees aren't included.
Why the fee isn't the whole cost
Exchanges compete loudly on their trading fee, but the quieter cost is the spread — the gap between the price you can buy at and the price you can sell at. A "zero-fee" or low-fee exchange can still be expensive if it quotes a wide spread, because you cross half that spread every time you enter or exit. For a realistic picture, always add the spread to the fee.
How it works
The trading fee is simply your trade amount × the fee rate. The spread cost is your amount × half the spread percentage for a one-way trade, doubled for a round trip (buy then sell). Add them together and you get the all-in cost — and dividing that back over your trade amount gives the effective rate you're really paying.
